50041 Bulwark derailed when entering Paddington station at around 60 mph with the 'Night Riviera'. After leaving Reading the train ran normally until around
two miles from Paddington, where instead of slowing to observe the 60 mph speed restriction on the Up Main line, it continued at speed. It was still travelling
in excess of 65 mph when it entered a crossover half a mile outside the station and over the maximum permitted speed which was 25 mph and derailed
